Transcript Request

  • To Request your "Official" Transcript - Download this form to your computer, fill in the information and save the changes.  

     

    BUT what is crucial to the process is that you sign the form.  

    This can be done a several ways; 

    1. By printing and signing this form and then returning it by fax 1.306.373.6968, Attn. Registrar's Office
    2. Printing, signing and then scanning the signed form back onto a return email 
    3. or Printing, signing the form and mailing it back to us with your payment.  
    4. If you have enabled the security feature on your computer that provides an electronically encoded secure signature you can use that option for a signature.

     

    The service charges for transcripts are printed on the attached form and payment can be made by mailing a cheque with your form or by credit card over the phone by calling our toll free number which is included on the form.  Do not send your bank or credit card information on this form.

     

    Please note that in most cases, when one is required to provide an "Official Transcript" it is only accepted when sent directly from the issuing Educational Institution to the Organization making the request, and not from the student themselves.
